Protocol

The 6-Minute Walk Test (6MWT), standardized according to ATS (American Thoracic Society) guidelines, measures clinical and functional exercise capacity. It is widely used in populations with chronic lung disease, heart failure, and the elderly. Preparation: The test corridor should preferably be 30 meters long, flat, indoors, and sheltered from airflow. Markers are placed every 3 meters. The participant arrives at least 10 minutes before the test and rests in a seated position. Contraindications are checked: resting heart rate >120, systolic blood pressure >180 mmHg, severe cardiac symptoms. Test Procedure: The participant is given a standardized instruction before the test: ‘In this test, you will walk as fast as possible for 6 minutes without running. Turn around at the end of the corridor and continue. You may slow down or stop if you become tired, but try to resume as soon as possible.’ The test administrator starts a stopwatch. Laps are counted during the test. Standardized motivational phrases are given each minute. Data Recording: The total distance walked in 6 minutes is recorded in meters. Oxygen saturation, heart rate, RPE, and Borg Dyspnea Scale values are recorded at the start and end. Reference Equations: Expected distance in healthy adults; male = 1140 − (5.61 × BMI) − (6.94 × age); female = 1017 − (6.24 × BMI) − (5.83 × age) (Enright & Sherrill, 1998). Post-Test: The participant sits down, vital signs are re-measured, and symptoms are assessed. Results are compared to predicted values and expressed as a percentage.

Practical Applications

The 6MWT is considered a gold standard in cardiopulmonary rehabilitation, heart failure management, and functional capacity assessment in older adults. It is used in repeated measurements to monitor treatment effectiveness.

Limitations

Ceiling effects may occur in healthy young individuals. Motivation and verbal encouragement affect protocol compliance. It is not a direct alternative to laboratory VO₂max measurement.
